How does this make any sense, an independent investigation showed that the police officers acted reasonably and it was caused by a pre-exhisting condition and the city still has to pay 3 mill?

Because they still could have gone to trial and risked a bigger judgment.
The city likely had some sort of insurance policy and they could have pushed for a settlement for that reason.
