signup forms online?

hey, I'm trying to get started organizing all the NBA Bloggers to do the previews this year, and I'd rather not fight the battle of copying and pasting all those emails, names, URLs, etc.

can anyone point me to a website that collects sign-up data and stores it in a table that would keep things organized?  (for free please)

for example, I looked into surveymonkey.com, but the whole idea behind that is to compile anonymous data, not to keep things organized

doesn't have to be too complex, I'm really only looking to capture 4 or 5 data fields
