I'd have little problem with picking him at #28 if we decide to keep the pick. The much talk about salary cap hit and guaranteed contract is honestly fairly negligible at that point. If he's best player available that point, I see little reason not to go for him.

I wouldn't pick him in the first round, no way.  Using one of our 2 second rounders especially the lower of the 2?  Sure...

I'm all in favor of using a second-round pick on Upshaw.  Hopefully a non-guaranteed contract would be a powerful inducement for him to clean up his act and live up to his potential.  I also think there are several NBA teams other than the Celtics looking to take a flyer on Upshaw in the second round.  Here's how I would beat them to the punch:

Celtics trade 28+45 to Timberwolves for 31+36.  They would then be in position to draft Upshaw with the first pick in the second round. The 36 could be used for a super athletic prospect like Pat Connaughton or Richaun Holmes.

With a second round pick, you are basically auditioning a prospect without making a long-term commitment.  It's a low risk, potentially high reward approach.

To be honest, I'd be more concerned about heart problem than drugs. Not saying that's not a huge problem, but at least he'd have a shot at battling drugs. Depending on heart issue, it could very well be that he just can not play.
If the heart allows, I would certainly use a secong rounder on him.
