2 adult ones came to mind.

Omar from "The Wire" and Charlie in "Lost".
Omar's death was perfect.  The way it would really happen versus what viewers would expect.

TP, I think I was like 8 or 9 when I first watched Old Yeller and his death, and the way his death happened, hit me so hard. I had phobias about rabies for years  :-\

Probably didn't affect me in the way it's asked but a death that really surprised me was Kevin Spacey's character in LA Confidential.  He was billed as one of the main stars but when he got killed there still felt like a lot of movie left.  I just didn't see it coming.

It was a long time ago and not a great movie IMO, but I gave some credit to the writers of “To Live and Die in L.A.” for offing one of the main characters fairly early on.

The curb stomping in “American History X” effected me like none other I’ve seen on screen.
